Education Requirements:Minimum of Associate's Degree in major field of study related to early childhood (social work, psychology, sociology, nursing, pre-K education, early childhood development, etc.) OR 12 semester hours or 18 quarter hours in these fields.
Essential Job Functions:Assist pregnant women and families with children age birth to three who are at risk for developmental delays or have been diagnosed with oneConduct home visits in order to screen children, refer to providers, provide support and educationCreate and implement an IFSP - Individualized Family Service PlanCoordinate services for referrals, evaluations, monitoring, advocacy etc.Develop, update and maintain records on clientsAttend all required training in-service sessions, HMG and CRIS meetings, conferences, workshops etc.
Complete skills inventory with clinical supervisor's approval
Work Experience and Skill Requirements:Must be fluent in Spanish and English Minimum of two years experience working with children birth through 3 yearsHave a service coordination credential issued by Ohio Dept. of Health (or willing and able to obtain within one year of hire)Manage multiple tasks, organize excellentlyAble to work independently within a teamHave a valid Ohio driver's license and liability insurance Work Experience and Education Preferences:
