Education Requirements:
- Minimum of Associate’s Degree in major field of study related to early childhood (social work, psychology, sociology, nursing, pre-K education, early childhood development, etc.)
- OR 12 semester hours or 18 quarter hours in these fields.
Essential Job Functions:
- Assist pregnant women and families with children age birth to three who are at risk for developmental delays or have been diagnosed with one
- Conduct home visits in order to screen children, refer to providers, provide support and education
- Create and implement an IFSP – Individualized Family Service Plan
- Coordinate services for referrals, evaluations, monitoring, advocacy etc.
- Develop, update and maintain records on clients
- Attend all required training in-service sessions, HMG and CRIS meetings, conferences, workshops etc.
- Complete skills inventory with clinical supervisor’s approval
Work Experience and Skill Requirements:
- Must be fluent in Spanish and English
- Minimum of two years experience working with children birth through 3 years
- Have a service coordination credential issued by Ohio Dept. of Health (or willing and able to obtain within one year of hire)
- Manage multiple tasks, organize excellently
- Able to work independently within a team
- Have a valid Ohio driver’s license and liability insurance
